If you're having trouble registering your sample, please submit this form, then go ahead and drop your sample at a FedEx location. Make sure to take a picture of the sample ID number on your tube, and the FedEx label with tracking number for your records.

I have taken a picture of my sample tube showing the sample ID, and the FedEx return label for my records.

We cannot accept samples from New York state(Required)

I understand that Peekaboo samples collected and mailed from New York state cannot be processed per NY state regulations. We will reject packages with New York state tracking information. All customers, including NY state residents, must collect and mail samples from another state.
